Renal Multiple-dose: after loading dose, CrCl >50 = 100% dose, CrCl ≤50 (no dialysis) = 50%; hemodialysis = 100% after each session. No adjustment for single-dose vaginal candidiasis.

Pregnancy High-dose (400–800 mg/day) first-trimester exposure linked to a distinctive pattern of congenital anomalies; avoid except in severe/life-threatening infection. Single 150 mg dose may also pose risk.
