- Blocks bacterial protein synthesis at the 50S subunit
- Generally bacteriostatic

- Azithromycin — Tissue-persistent macrolide for atypicals; convenient courses, but QT risk.
- Chloramphenicol — Broad antibiotic reserved for severe infections; aplastic anemia risk.
- Clarithromycin — Macrolide used in H. pylori and atypical infections; a strong CYP3A4 inhibitor.
- Clindamycin — Lincosamide for anaerobes and toxin suppression; classic C. difficile risk.
- Erythromycin — The original macrolide; also a motilin agonist used as a prokinetic.
- Linezolid — Oxazolidinone antibiotic for resistant gram-positives; a reversible MAOI (serotonin risk).
- Tedizolid — Once-daily oxazolidinone for acute bacterial skin infections, including MRSA; inhibits 50S protein synthesis.
- Telithromycin — Ketolide macrolide largely withdrawn over hepatotoxicity.
