NKCC1 inhibitor
NKCC1 cotransporter inhibition (inner ear — ototoxicity)
NKCC1 inhibitor — mechanism card
- The ubiquitous NKCC isoform; in the stria vascularis it maintains the endocochlear potential
- Loop-diuretic ototoxicity — usually reversible tinnitus and hearing loss, worse with rapid IV dosing, renal failure and aminoglycosides
- Ethacrynic acid is the most ototoxic of the class

Drugs with this mechanism (4)
- Bumetanide — Potent loop diuretic; more predictable absorption than furosemide.
- Ethacrynic acid — The only non-sulfonamide loop diuretic; also the most ototoxic.
- Furosemide — The prototypical loop diuretic; workhorse for edema and heart failure.
- Torsemide — Loop diuretic with better oral bioavailability than furosemide.
