Heparin-induced thrombocytopenia (HIT)
- Stop ALL heparin immediately
- Start non-heparin anticoagulant if likely HIT
- argatroban/bivalirudin or selected Xa drug
- Do not await confirmatory test if high probability
- Avoid platelet transfusion unless bleeding
- Delay warfarin until platelets recover
- Document heparin allergy clearly
