- TMP-SMX is preferred treatment
- Treat for 21 days in HIV-associated PJP
- Moderate–severe hypoxemia → add corticosteroid
- Alternatives depend on severity + G6PD
- pentamidine or clinda/primaquine
- Start ART promptly in HIV when feasible
- Begin secondary prophylaxis after treatment
