- Check iron deficiency + other causes first
- Replete iron before/with ESA when needed
- ESA if significant anemia persists
- Goal is transfusion reduction—not normal Hb
- Higher Hb targets increase vascular risk
- HIF-PH inhibitors are another option in some pts
- Monitor Hb, iron indices + BP
