Epoetin alfa

EPOGEN

What it isRecombinant erythropoietin stimulating marrow red-cell production; anemia of CKD and chemotherapy.

Why this oneIts shorter action than darbepoetin permits finer titration but requires more frequent administration.

What limits it⚑ Chasing a normal hemoglobin increases death, stroke, and thrombosis; target symptom relief and ensure adequate iron.

FDA label

BOXED WARNING ESAs increase the risk of death, myocardial infarction, stroke, venous thromboembolism, thrombosis of vascular access, and tumor progression or recurrence. Use lowest dose to reduce RBC transfusions; in CKD do not target hemoglobin >11 g/dL; in cancer use only for chemotherapy-induced anemia and discontinue after the chemotherapy course; DVT prophylaxis recommended perisurgery.
