The metabolite that burns the bladder
- Acrolein: reactive aldehyde metabolite of cyclophosphamide/ifosfamide, excreted and CONCENTRATED in urine
- Direct urothelial injury → hemorrhagic cystitis; dose- and contact-time-dependent

- Defenses: mesna (binds it), hyperhydration and frequent voiding (dilute it, shorten contact)
