DHP-1 inhibitor

• Inhibits renal dehydropeptidase-1, the brush-border enzyme that otherwise hydrolyzes imipenem in the proximal tubule
• No antibacterial activity of its own — it protects the partner drug and restores its urinary concentrations
• Also limits proximal-tubule accumulation of imipenem and the nephrotoxicity that follows

Drugs with this mechanism (1)

• Cilastatin — Renal dehydropeptidase inhibitor that prevents imipenem breakdown; has no antibacterial activity of its own.
