Titration May increase after 1 week of the initial dose.

Renal Use with caution in significantly impaired renal function.

Hepatic Use with caution in significantly impaired hepatic function.

Documented safety not available for doses above 80 mg/day for periods longer than 2 weeks.

Instructions

• If a dose is missed, continue the normal dosing schedule
• Discontinue if efficacy not achieved within 2 weeks or if side effects require doses below 80 mg/day

Contraindications

• Infants less than 6 months of age
• Nursing mothers
• Unstable cardiovascular status in acute hemorrhage
• Myasthenia gravis
• Glaucoma
• Obstructive uropathy
• Obstructive disease of the gastrointestinal tract
• Severe ulcerative colitis
• Reflux esophagitis

Cautions

• Worsening of cardiovascular conditions
• Heat prostration (fever, heat stroke from decreased sweating) in hot environments
• Psychosis and delirium in anticholinergic-sensitive patients (elderly, mentally ill); resolves 12–24 h after discontinuation
• Myasthenia gravis: overdose may cause weakness/paralysis
• Incomplete intestinal obstruction (diarrhea may be an early sign, esp. ileostomy/colostomy)
• Toxic megacolon risk in ulcerative colitis and Salmonella dysentery
• Urinary retention in prostatic hypertrophy
• Caution in hepatic/renal disease and the elderly

Adverse reactions

• Dizziness
• Dry mouth
• Blurred vision
• Nausea
• Somnolence
• Asthenia
• Nervousness

Pregnancy Pregnancy Category B; use only if clearly needed.

Lactation Contraindicated while breastfeeding; excreted in breast milk (risk of serious adverse reactions in the infant).
