What it is Long-acting muscarinic antagonist; COPD maintenance.
Why this one Rapid plasma hydrolysis distinguishes it from other LAMAs and limits systemic anticholinergic exposure.
What limits it It is twice-daily maintenance, not rescue; urinary retention and narrow-angle glaucoma remain the key anticholinergic hazards.

FDA dosing Population Start Target Max
COPD — maintenance treatment of bronchospasm Adults 1 oral inhalation 400 mcg BID 400 mcg BID (~12 h apart) 1 inhalation BID
Renal No adjustment needed
Hepatic Undefined
Instructions For oral inhalation only; one inhalation morning and evening, approximately 12 hours apart. Not for acute bronchospasm; not a rescue medication. Not indicated in pediatric patients (COPD does not normally occur in children).
Contraindications Severe hypersensitivity to milk proteins
Cautions Not for acute use/rescue therapy. Paradoxical bronchospasm — stop and consider other treatments if it occurs. May worsen narrow-angle glaucoma — use with caution; consult physician immediately if eye pain, blurred vision, halos. May worsen urinary retention — caution in prostatic hyperplasia or bladder-neck obstruction. Immediate hypersensitivity reactions (anaphylaxis, angioedema, urticaria, bronchospasm) — discontinue at once.
Adverse reactions Headache Nasopharyngitis Cough (1–3%): diarrhea, sinusitis, rhinitis, toothache, fall, vomiting
Pregnancy No adequate human data; no adverse developmental effects in animals at 15–20x the human inhaled dose (effects only at very high oral multiples).
Lactation No human data; present in rat milk — likely present in human milk.
Principal risk Anticholinergic effects; twice-daily dosing
Cost, est. cash $200–$900/month
Generic entry 2029 est.
