- Stimulate red-cell production
- CKD anemia + selected chemotherapy anemia
- Require adequate iron availability
- Goal is transfusion reduction, not normal Hb
- Higher Hb targets ↑ stroke/thrombosis risk
- Hypertension can worsen
- Avoid inappropriate use in curable malignancy
