- Check ammonia with encephalopathy—not routinely
- L-carnitine is used for severe toxicity/overdose
- Treat contributing infection, fasting or urea-cycle risk
- Normal LFTs do not exclude valproate hyperammonemia
- Severe encephalopathy may require dialysis
